What are the safety lockout procedures to be performed by the power plant operator before doing site electrical loads preventive maintenance?

I directed and managed all aspects for more than80+ personnel that worked for me. This was not only during work hours but also because of the remote location and no other support available I managed24 hr support that included offices,supply rooms, and a kitchen with dinning area. My main focus was operating / maintaining and repairing all assets buildings, power plants, water treatment plants, waste water treatment plants and more than80 back up emergency generators ( from12 to450 KVA,50 Hz,3ph. ) for more than40 personnel in the sites. I had an excellent working relationship with the construction contractor and American Corps of Engineers (COE).

1. Isolate the maintenance portion

2. Discharge the portion through grounding switches or external ground

3. Lock the control panel and keep the keys with the supervisor.

4. Sign the work permit

5. Special care must be taken of induction from nearby lines or live parts.

For major electrical services ensure the following:

1. Change control is in place - who is doing what and when - Scope of work

2. Ensure that all parties impacted by shutdown are aware.

3. Lock out relevant switchgear with a physical lock, multiple locks if more than one technican working on system.

4. Signange notifiying of work in place.

5. Sign on relevant panel indicating safe for work.

6. Ensuring all parties are accounted for before recconecting service.

 

 

Modern machinery can contain many hazards to workers, from things like electrical, mechanical, pneumatic or hydraulic sources. For example a typical industrial machine may contain things like hot fluids, moving presses, blades, propellers, electrical heaters, conveyor belts with pinch points, moving chains, ultraviolet light, etc.

Disconnecting or making safe the equipment involves the removal of all energy sources and is known as isolation. The steps necessary to isolate equipment are often documented in an isolation procedure or a lockout tagout procedure. The isolation procedure generally includes the following tasks:

  1. Identify the energy source(s)
  2. Isolate the energy source(s)
  3. Lock and Tag the energy source(s)
  4. Prove that the equipment isolation is effective

The locking and tagging of the isolation point lets others know not to de-isolate the device.

there are many methods to lockout like:

    Lockouts and tagouts do not by themselves re move power from a circuit. An approved procedure is followed when applying a lockout/tagout. Lockouts and tagouts are attached only after the equipment is turned OFF and tested to ensure that power is OFF. The lockout/tagout procedure is required for the safety of workers due to modern equipment hazards. OSHA provides a standard procedure for equipment lockout/tagout. OSHA's procedure is:

  1. Prepare for machinery shutdown.

  2. Machinery or equipment shutdown.

  3. Machinery or equipment isolation.

  4. Lockout or tagout application.

  5. Release of stored energy.

  6. Verification of isolation.

1. Conduct proper risk assessment of the work involved. Review work method 

    statement , scope of work  etc.

2. Prepare isolation procedure for the affected euipment/process.

3. Coordinate isolation procedure to shift supervisor in the power plant control 

   room.

4. Control room operator to coordinate isolation of the correct equipment in the

   field by the isolation authority. ( i.e. verify correct equipment to be isolated in

   the field by verifying in the isolation procedure and work permit request)

5. Conduct  safe shut down the affected equipment. 

6. Apply proper lock out tag out of the equipment.Isolate the equipment safely by

   following relevant safety methods. Conduct  Pre start safety analysis before

   isolation of the affected equipment. follow step

   by step point of isolation as per isolation procedure.

7. Ensure zero hazardous energy is achieved before equipment is hand over for

   preventive maintenance. Else an approved procedure maybe required if

   working on energised equipment is a requirement.

8. Always follow Company site Health Safety and Environmental procedures. 

9. Be SAFE and always stay SAFE. SAFETY is everyones responsibility.
