Why should always be related to finding a solution. This part of making proper and stable solutions to problems. For example why did the Circuit breaker trip. is it due a fault or overload. Therefore the why will help you analyse properly to come up with a proper solution.

Plan is closely related to ''HOW'' how are we going to reach the organization mission and vision, or how are we going to satsfy the customer .. a plan can be for anything just asking how and you will plan right away.
Improving and desire is closely related to ''WHY'' why are we creating this project, why to open new branch to hire someone why to open this business.
All come from a burning desire inside us to do something or improve something we already have then comes the how and the plan

It describes the business case, it identifies the need that this project should fulfill .
It is "WHY" we are planning for this project ...
